If you have concerns about the accuracy of our Primary Election and/or saw irregularities during the election, here is your chance to help the candidates you voted for by filling out either Affidavit as applicable. Examples of irregularities may include the following:
- While attempting to vote in person, you were told you already voted in person
- You received multiple absentee ballots either addressed to members of your household or to people not part of your household
- Pollworkers not requiring IDs in order to vote
- Pollworkers not scanning IDs to verify registry (some counties reported their network for verifying IDs were down
- Political signs at the polling place
- Any other issues
Instructions:
Step 1. Click on image to open and download the PDF file to complete the applicable Affidavit. Upon completion, print it out for notarization.
Step 2. Have the Affidavit notarized. Banks, post offices, churches, small businesses, and some grocery stores have notaries.
Step 3. Take a picture of the completed form with a smartphone or use a scanner to e-mail it to affidavit@auditthevoteiowa.com so we can tabulate the responses. Please mail the notarized Affidavit to P.O. BOX 524, Bettendorf, IA 52722 for
